Join us to discuss how we can work together to continue to improve the experience of leaving hospital (hospital discharge).
We are partnering with Ruils, St George’s Hospital and other local organisations to take a look at what has improved in experiences of hospital discharge since we spoke to local carers and the hospital has implemented the Carers’ Hospital Discharge Toolkit.
