Thank you to everyone who has contributed to our work this year. Our annual report celebrates what we have done together and the difference that this has made.
You can take a glance through the examples of how we have influenced improvements in how carers are involved when someone leaves hospital and others ways that we have been making sure that the voice of the community is heard when the NHS and social care are planning and running their services.
Highlights include:
- Over 1100 people involved in sharing experiences of health and social care services.
- Over 31 people volunteered their time to help strengthen the voice of the community in health and social care. Read more about Oksana and Fatima's experience.
- All of the stakeholders that we asked said that we had increase their insight/knowledge of the community in Wandsworth and that we had empowered the community with information to reduce health inequalities.
- 11 people we spoke to who were in charge of health and social care services said that they had committed to improvements after speaking to us. A further 4 were intending to.
- 8 hospitals across London are already implementing a toolkit to improve the involvement of carers in hospital discharges and St George's Hospital has committed to start using it.
- We have been making sure that people with sight loss are being heard, that the community can tackle childhood obesity and that mental health support for new parents improves.
